Curriculum Implementation Survey Toolkit
A curriculum rollout isn't finished when the materials are distributed. This toolkit helps you find out where implementation is actually stalling — weeks in, when you can still do something about it.
The survey distinguishes between Actionable Drivers — clarity, usability, support, feasibility — things you can address right now — and Informational Drivers — alignment and teacher confidence — signals that tell you where longer-term investment is needed. That distinction changes how you respond to results and keeps you from trying to fix confidence when the real problem is unclear expectations.
Grounded in the Concerns-Based Adoption Model (CBAM/AIR), RAND's Assessment of Instructional Resources Study, and implementation science research on adoption and fidelity. Designed for administration 4–8 weeks after a major rollout milestone, with guidance on re-administration timing and what to watch for as implementation matures.

About EvaluationToolkits.com
EvaluationToolkits.com offers downloadable survey toolkits for K–12 schools and youth-serving programs. All tools are created by Rebecca, a PhD in assessment and evaluation with 25+ years of experience working in and with schools, districts, research centers, and nonprofit programs.

Frequently Asked Questions
What's included in this toolkit?
Every toolkit includes five components: a ready-to-administer survey (print or digital), an admin guide with research foundation and interpretation guidance, an auto-analysis spreadsheet that calculates scores and flags areas needing attention, a one-page results summary in both plain and branded formats, and a PowerPoint results template for presenting findings to staff, boards, or your superintendent.
Can I use this toolkit more than once?
Yes — that's the intention. Run the same survey each cycle and you'll build trend data over time. Showing your superintendent or board that a score moved from 2.8 to 3.9 over two years is more useful than any single data point. Just clear the previous year's responses from the spreadsheet and you're ready to go again.
What software do I need?
The auto-analysis spreadsheet requires a desktop version of Microsoft Excel (Mac or PC). The survey and admin guide are Word documents compatible with most word processors. The results presentation is a PowerPoint file. None of the tools require special software beyond standard Microsoft Office.
Does this work for any size school or program?
Yes. The surveys work for small teams of 8–10 and larger organizations with 50+. The analysis spreadsheet handles any sample size and notes when results may be less reliable due to a small number of responses.
Is this licensed for one site or my whole organization?
Each purchase includes a single-site license for one school building or program location. For use across multiple campuses or sites, contact rebecca@evaluationtoolkits.com about multi-site licensing — volume pricing is available.
Are these surveys validated research instruments?
These are practical perception surveys designed for program improvement, school decision-making, and funder reporting. They are not psychometrically validated for high-stakes research or personnel evaluations. Results should always be reported as stakeholder perceptions — for example, "82% of respondents agreed or strongly agreed that..." That framing is accurate, honest, and appropriate for sharing with superintendents, boards, and funders.
Can I see how the toolkit works before I buy?
Yes — the free Staff Feedback Survey Toolkit is a complete toolkit in the same format as every paid product in the library. Download it, use it, and you'll know exactly what you're getting before you purchase anything else.
Is this licensed for one program or one organization? Single-program use. If you run multiple sites and want to use it across all of them, reach out about a site license — volume pricing is available at rebecca@evaluationtoolkits.com.
